Skills Older Adult Day Services
Skills Senior Day Programs are designed for older adults with developmental disabilities and emphasize respect, dignity, and participant safety.
Skills Older Adult Day Program offers training and support for older adults with developmental disabilities. Clients are invited to participate in a variety of activities designed to meet their individual interests and needs.
Skills Older Adult Day Program offers a range of choices for each individual to participate in activities appropriate to his or her age and interests. Skills places emphasis on achieving personal fulfillment, fostering independence in making life decisions, and interacting with peers. Group activities include community outings, hobby exploration and health maintenance.
Skills Older Adult Day Program focuses on three key wellness areas: daily living activities, health and fitness, and community interaction.
Daily Living Activities
Skills Daily Living Activities program gives clients the opportunity to improve their knowledge about a variety of topics. Group discussion develops social interaction and strengthens communication skills.
Leisure activities include arts and craft projects, gardening, cooking/baking, group sing-a-longs, bingo, sharing favorite movies, and discussions about current events.
Health and Fitness
Skills Health and Fitness program encourages older adults to stay active and healthy by participating in physical fitness exercises. Special attention is given to meeting each individual’s needs while participating in individual and group exercise activities.
Older adults, including those with limited mobility, enjoy a well-planned experience to enhance or maintain physical fitness, muscle strength, and coordination. Additional active games, such as bucket basketball, ring tossing, penny pitching, horseshoes, and darts, increase enjoyment and add variety to planned physical conditioning activities.
Other activities that support health and physical fitness include music therapy sessions, which use rhythm band instruments to enhance coordination skills, and community activities, such as bowling and swimming, at local recreation facilities.
Skills Health and Fitness Program also focuses on developing and maintaining self-care skills. Discussions about grooming and nutrition are held regularly.
Community Interaction
Skills older adults participate regularly in community-based activities, such as shopping, dining out, picnics, and birthday parties. Community activities include live theatre presentations, bingo, visiting senior centers for lunch, boats rides, and more.
Eligibility Requirements
Adults with functional impairment who are 60 years of age or older or adults who are 18 years of age or older and have post-stroke dementia, Parkinsonism or dementia-related disease, such as Alzheimer’s or other organic brain syndrome.
Intake/Admission Procedures
Following referral for services, intake procedures begin with a tour of the facility and a meeting with the individual’s support team. Medical and/or psychological evaluations are arranged as needed. Upon receipt of authorization, an admission date os set and training program plans are discussed. The screening shall be completed within 60 days prior to admission.
Plan Process
A team meeting is held to develop an Individual Support Plan (ISP) for each individual that follows the philosophy of self-determination. ISPs are completed, reviewed, and updated for each participant on an annual basis. In addition, team meetings may be called by a team member at any time when revisions need to be made or when a significant problem arises that needs to be discussed.
State-Licensed Facilities
Skills Older Adult Daily Living Centers are licensed by the Department of Aging and Long-Term Living Title 6 PA Code Chapter 11 Regulations.
