Recovery is facilitated by relationships and environments that provide hope, empowerment, choices and opportunities that promote people reaching their full potential as individuals and community members.
The goal of the Mobile Psychiatric Rehabilitation Service to help individuals make their hopes and dreams for their life become a reality.
Skills works closely with local providers and community resources to identify opportunities for participants to become more active in the local community. Unlike site-based programs, this mobile service focuses on individuals whose goals are in-community, work-based or home-based and on individuals who may not be ready to benefit from site-based supports.
The Mobile Psychiatric Rehabilitation program’s core principles include:
- Self-direction: Individuals plan the direction and focus of the service.
- Individualized and person centered: Each individual’s plan is developed based on his or her assessed needs. The program will develop interventions based on those that will foster success.
- Empowerment: Individuals run this program and their own lives; Staff act as guides.
- Holistic: Staff works with the individual to address all of his or her needs, not just those focused on mental health.
- Strengths-based approach: Staff focus on an individual’s strengths, not his or her barriers, to develop goals based on what he or she is good at and what motivates him or her.
- Embracing peer supports: Staff help establish connections with those who know where each individual has been and where he or she can go.
- Fostering respect: Staff respect everyone in the program. Staff’s job is not to criticize or change people; it is to embrace who each person is as an individual.
- Encouraging self-responsibility: People are responsible for their own lives. Staff cannot “make someone better” or force change — Change must come from within. Staff cannot be responsible for someone else’s choices, but instead offer support and encouragement as an individual makes his or her journey.
- Hope: Staff believes everyone can learn and grow. Staff must be hopeful even when the individual is not.
Individual Recovery Plans (IRPs) are developed together by the individual served and Skills mobile psych rehab worker based on the identified needs and desires of the individual. IRPs include skills building and resource acquisition in the domains of living, learning, working, and social environments. Staff have access to and training in the use of many innovative tools to support recovery. Mobile psych rehab staff work side-by-side in community settings to help individuals served complete goals that prioritize community integration and resource acquisition.
Mobile Psychiatric Rehabilitation serves individuals who:
- May currently be unable to attend a site-based service.
- Set a goal which needs substantial community exploration or practice.
- Have chosen a goal that indicates the preferred site for service and support is within the community.
This referral program is for individuals ages 18 or older with a qualifying diagnosis as outlined in Health Choices program standards; the person wants to receive services; and as a result of the mental illness, the individual has moderate to severe functional impairment that interferes with role performance in vocational, educational, and/or self-maintenance domains. This service is intended to be short-term and to help participants choose, achieve and keep their goals.
Available throughout Centre County, staff are available Monday through Friday with some evenings and weekend hours as needed.
This program is funded through Community Care Behavioral Health.
