Skills of Central Pennsylvania, Inc. is committed to protecting the privacy of Protected Health Information (PHI). PHI is identifiable health information, including demographic information, collected from the consumer or created or received by a health care provider that relates to:

  • Past, present, or future physical or mental health condition.
  • The provision of health care to the consumer.
  • The past, present, or future payment for the provision of health care to the consumer.
  • Skills will inform the consumer of our practices for collecting, using, securing, and sharing PHI on or before the first day of services.

Skills maintains physical, electronic, and organizational safeguards to protect consumers’ information. On an ongoing basis, Skills will monitor, review, and revise our Privacy Practices to ensure the privacy of our consumers’ PHI.

Skills reserves the right to make changes in our Privacy Practices due to changing circumstances. The new terms of our Privacy Practices will become effective for all PHI that we maintain. Skills will notify all affected consumers in writing of changes to our Privacy Practices.
