Supported Living Services
What should I know?
Supported Living Services through Skills of Central PA, Inc. enhance the ability of an individual to lead a self-determined life style by providing the support and information necessary to build self-esteem, assertiveness, and to make informed decisions.
Person Centered
Skills’ Home and Community Support Services are designed to address individual needs.
This program creates a sense of empowerment that allows an individual to make choices and achieve a desired life-style.
Supported Living Services
   
Refferral Procedures
Following a referral for services, an interview with the individual and/or family is held to determine his/her wants and needs.  Once authorization for service has been approved, a start date is set based on the results of the referral information and the interview. 
A Home and Community Support Assistant provided by Skills will work with the person to encourage greater independence, and to enhance his or her ability to live and participate in the community.  The individual's team meets regularly to review progress, explore new opportunities and problem solve.
 
Areas of Support *
Medical – Address all aspects of Medical concerns such as scheduling, attending and transporting individuals to medical appointments. Medication education and support will also be provided as needed.
Supported Living
Community Connections- Involving individuals in various community activities in order to broaden opportunities and relationships, including volunteer experiences.
Financial Development- Support individuals with budget related issues such as saving to purchase larger items, banking, bill paying and shopping.
Maintaining a Household- Basic support with cleaning, laundry and general maintenance concerns.
Emotional Support- Fostering communication and problem solving skills between the individual and those involved in their lives.
Transportation- Provide transportation as needed and explore public transportation options such as training on local bus routes, using a taxi and/or local van services.
Health and Safety- Address issues of health, nutrition and exercise, teach menu planning, food shopping, preparation skills and personal care as well as community, home and personal safety.
Self Advocacy- Encourage and support individuals to speak for themselves and make their own choices.
* Other program areas can be discussed and tailored to the individual’s wants and needs.
